Health Homes Care Manager

Position Summary


Care Managers provide outreach and enrollment services to children presumed to meet eligibility requirements of New York State’s Children’s Health Home program. They administer assessments and enter documentation that confirms eligibility and identifies areas requiring supportive services. They work closely with children, families and service providers to create a comprehensive Plan of Care, identify additional service providers as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan. Care Managers document progress and arrange for eventual transition between health homes as necessary, or from health homes to other levels of service following changes in eligibility.


Primary Duties and Responsibilities:


Conduct field, phone and mail outreach to potential care management clients and/or legal guardian

Conduct outreach and provide clients and internal/external providers with an overview of the Care Management service model

Document all case activity, including outreach, consent development and assessment, plan development, client progress and transition arrangements.

Become certified (annually) and administer CANS-NY, updating periodically.

Develop a comprehensive family-driven, youth-guided Plan of Care.

Inventory and coordinate existing services relevant to the Plan of Care, identifying and securing additional services as appropriate.

Convene and conduct meetings to review progress, update POCs, and confirm continuing client eligibility.

Provide education to family and child concerning the nature and desired effect of services, along with information concerning conditions being treated.

Assist families and children in the acquisition and maintenance of public benefits e.g., financial, educational, social, and community services.

Provide Health Home services for children at medium or high acuity, including at least two services per month, with at least one of those conducted face to face.

Assure responsible transition of client service into and out of Health Home care, between child and adult health homes, and between inpatient and community care as appropriate.

Participate in mandatory and optional training, including but not confined to: CANS-NY administration, person-centered planning, trauma-informed care, meeting facilitation, safety in the community, engagement/outreach, LGBTQ and cultural competency.

Additionally, candidates will demonstrate knowledge and understanding in EI eligibility, state and Federal EI regulations, principles of family-centered service, and the scope of service available under the EI program.

Participate in regular supervision.


Qualifications and Competencies:


? Bachelors of Arts with two years of relevant experience (psychology, social work, etc.) OR

? Registered Nurse or Licensed Practical Nurse with two years of relevant experience OR

? A Master’s Degree in related field (License preferred)

? Early Intervention experience a plus

? Bilingual/Spanish is a plus


In addition, the candidate must demonstrate the following Competencies:


? Excellent writing, communication, and organizational skills

? A passion for serving children and families with special needs

? Ability to travel to community-based agency offices, participant’s communities, and homes within assigned catchment area

? Ability to receive feedback to professionally grow and/or improve

? Proven self-management abilities, including meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements
