Director Of Case Management

DIRECTOR OF CASE MANAGEMENT (CANS certified a must)

This position is part time  (20 hours per week)with possibility of leading to full time ,working within our Health Homes Program (open to 20 hours Health Homes -20 hours evaluations) 

 

JOB DESCRIPTION: Hire, train and supervise Early Intervention and Medicaid Service Coordinators (MSCs) who assist children and their families to obtain access to services and support appropriate to their needs. In addition, Director must be able to perform all MSC duties as needed.

 

Overview of the Supervision of Service Coordinatorís responsibilities associated with the Individual Service Plan (ISP):               

 

  • Developing, implementing and maintaining a child/familyís ISP

  • Assessing the need for and facilitating the access to social, medical and support services              

  • Record Keeping

    Educational and Experience Requirements:

  • Bachelorís degree in Social Work required, Masterís degree preferred

  • At least two (2) years prior experience working with the Developmentally Disabled population

  • Experience working as a Medicaid Service Coordinator   

  • Required Knowledge, Skills, Abilities and Miscellaneous Items:

  • Hiring, training and supervising staff

  • Ability to use professional judgments and assess the needs of individuals

  • Recognizing and addressing health and safety issues

  • Facilitating meetings

  • Highly organized

  • Proficient computer skills- Excel a must

  • Experience with Early Intervention and Medicaid

  • Confidence and ability to work independently

  • Office for People with Developmental Disabilities (OPWDD) knowledge and experience

  • Bilingual a plus

  • Must have own car- travel is necessary

  • Willingness to obtain further training

    Training Requirements set by OPWDD:

  • Certificate of past attendance or attendance at an OPWDD approved Core service coordination training program within 3 months of assuming MSC responsibilities

  • Annual MSC professional development training

  • Minimum of 15 hours of professional development training annually for first three (3) years and 10 hours annually thereafter.

  • Child & Adolescent Needs and Strengths (CANS) training with a passing grade of 80% or higher
